I give permission for my child to appear in camp photos or videos used for training or promotional purposes.

I understand that hockey is a contact sport and involves inherent risks, including but not limited to falls, collisions, puck impact, equipment failure, and other potential injuries. I acknowledge and accept these risks on behalf of my child.

I agree to release and hold harmless Darrin Madeley, camp staff, coaches, volunteers, and the facility from any liability for accidental injury or property loss that may occur during camp activities, except in cases of willful misconduct.

In the event of an emergency where I cannot be reached, I authorize camp staff to secure and consent to medical treatment for my child. I agree to be financially responsible for any resulting medical expenses.

I confirm that my child is covered under a personal or family health insurance plan.

The camp reserves the right to dismiss any camper whose behavior is unsafe or disruptive, without refund.
